A failure coding agents keep rediscovering, session after session, starting from zero each time. One line in the instructions they read stops it.

This run is in don’t-ask mode: tools outside the allowed set are denied, not prompted. Prefer Read/Grep/Glob and simple single commands, and report BLOCKED instead of retrying variants.

How it did

Gathering evidence: results appear once 5 people have adopted this fix and have enough sessions on both sides to compare. Until then, no numbers, because a result from one or two people isn’t a result.

How it’s measured: on each adopter’s own machine, trail compares sessions in the same repo and the same permission mode before and after the rule was added. Only counts leave the machine; never code, prompts or file names.
